ADVANCE REGISTRATION / NO REFUND / PREREQUISITE POLICY:
All Group Classes require a minimum 2 week (14 Days) advance registration to be guaranteed a specific date for skills instruction
and testing (Skills Lab) at your location or ours. You are required to schedule Skills Lab within 30 days of purchase date of the course to get the Skills Lab included with the purchase price. If not purchased within the 30 days from purchase date you will lose the ability to attend the Skills lab for the original purchase price and will have to purchase the Skills Lab again to be able to attend the Skills Lab. No one will receive an AHA Card without completing the Skills Lab portion.
Heart Saviors, Inc. will not give any refunds for any reason, once course or any materials are purchased. All transactions are final. You may only have the option to re-schedule the Skills lab date only once unless a written email has been accepted by Heart Saviors, Inc. A minimum of 24 hours notice with instructor approval is needed to change the date of your class. If you are registered for a class but cannot attend: Heart Saviors, Inc. must be notified in writing by email no later than 7 days prior to the class you are registered to attend to be able to reschedule. Failure to notify will result in the forfeit of any obligation by Heart Saviors, Inc. No shows forfeit the entire fee. If you are more than 30 minutes late for the Skills Lab you will not be admitted to the class, you will be allowed to reschedule one time for no additional charge. Any registrations received within 2 weeks prior to any classes are not entitled to reschedule the class dates. All certificates, classes, skills training, books, and e-learning class materials are non-refundable and non-returnable, no exceptions. Heart Saviors, Inc. reserves the right to cancel or limit the time of the class without any prior notice. Heart Saviors, Inc. is not responsible for any travel related expenses incurred, including but not limited to: Airfare, Hotel, Taxi, Parking, Rental Car, Food etc. Your certification is non-refundable, non-transferable. Heart Saviors, Inc. is not a Job Placement Agency. We neither provide jobs, nor guarantee jobs. Heart Saviors, Inc. simply provides certifications for ACLS, BLS, Heart Savers CPR, AED, First Aid, Pediatric First Aid, and continuing education services.
Should the class for which you are registered be cancelled or rescheduled by Heart Saviors, Inc., you will be able to transfer to
another class day other than the one chosen by Heart Saviors, Inc. If you transfer to another date other than the one assigned, you forfeit your right to any future transfers, no exceptions.
By submitting this form I confirm that I have read and meet all of the medical background pre-requisites, am at least 18 years of age, and completely understand and agree with the Heart Saviors, Inc. policies and procedures. Noncompliance of any these polices will forfeit any obligation by Heart Saviors, Inc.

*** CARDS MUST BE PICKED UP AT INSTRUCTOR LOCATION ON THE DAY OF THE CLASS, OR AT THE MAIN OFFICE IN TORRANCE, CALIFORNIA - NO CARDS WILL BE MAILED TO STUDENTS, CARDS CAN ONLY BE PICKED UP IF THEY ARE PRINTED, STUDENT MUST COME BACK TO GET THE CARD IF IT IS NOT READY, PLEASE CONTACT US TO VERIFY THAT YOUR CARD IS READY
